Position Summary

We’re seeking a compassionate and clinically skilled Registered Nurse to join our home health team, providing skilled nursing care to patients in the comfort of their own homes. In this role, you will assess patient conditions, develop and implement individualized care plans, and coordinate with physicians and the interdisciplinary care team to promote patient recovery, independence, and quality of life. The ideal candidate will be a self-motivated clinician who thrives working independently, communicates clearly with patients and families, and is committed to delivering high-quality, patient-centered care outside the traditional clinical setting.

Key Responsibilities

  • Conduct comprehensive in-home patient assessments, including OASIS assessments for Medicare-certified patients
  • Develop, implement, and update individualized plans of care in collaboration with the patient’s physician
  • Administer medications, wound care, injections, and other skilled nursing interventions as ordered
  • Monitor and document patient vital signs, symptoms, and overall condition at each visit
  • Educate patients and family members/caregivers on disease management, medication administration, and home safety
  • Coordinate care with physicians, therapists, medical social workers, home health aides, and other members of the care team
  • Recognize and respond appropriately to changes in patient condition, escalating to the physician or supervisor as needed
  • Complete and submit accurate, timely documentation in compliance with Medicare, Medicaid, and agency standards
  • Supervise and provide clinical direction to LPNs/LVNs and home health aides, as applicable
  • Ensure patient rights, dignity, and confidentiality are maintained at all times (HIPAA compliance)
  • Participate in case conferences, care plan reviews, and interdisciplinary team meetings
  • Provide patient/caregiver education on disease processes, prevention, and self-care management
  • Respond to on-call/after-hours needs on a rotating basis, as required
  • Maintain compliance with all state and federal home health regulations (AZDHS, CMS Conditions of Participation)
  • Manage a daily visit schedule efficiently, prioritizing patient needs and travel logistics

Qualifications

  • Active, unencumbered Registered Nurse (RN) license in the State of Arizona
  • Graduate of an accredited school of nursing (ADN or BSN)
  • Minimum 1–2 years of clinical nursing experience; home health experience preferred
  • Current CPR/BLS certification
  • Valid Arizona driver’s license, reliable transportation, and current auto insurance
  • Knowledge of OASIS documentation and Medicare Conditions of Participation preferred
  • Strong assessment, critical thinking, and clinical decision-making skills
  • Ability to work independently with minimal supervision in varied home environments
  • Excellent verbal and written communication skills
  • Proficiency with electronic medical record (EMR) systems
  • Ability to pass a background check, drug screening, and obtain a fingerprint clearance card
  • Physical ability to perform patient care tasks, including lifting, bending, and transferring patients as needed
